HCFA 1500 / CMS 1500 Claim Forms is required to use to bill Insurance / Medicare and all federal insurance, now you need to use only the New 2012 Version Each form must have accurate content and conform to the Health Insurance Portability and Accountability Act (HIPAA). Developed in conjunction with all the governing agencies, including the National Uniform Claim Committee (NUCC), the National Uniform Billing Committee (NUBC), and the CMS Centers for Medicare and Medicaid Services, the Health and Human Services Agency, and the American Hospital Association Forms must adhere to strict printing standards that govern the layout, paper and ink. 

Quality forms offers the newest version of the CMS-1500 form, the CMS-1500 (02/12), which started being used in 2014 and replaced the CMS-1500 (08/05) version. The revised CMS-1500 claim forms are used to submit Medicare, Medicaid, and other health insurance claims. 
New CMS-1500 02/12 claim forms come in a variety of formats, including laser cut sheets for your standard laser or desktop printer, as well as in continuous feed format for your dot matrix paper printers. The most commonly used format is laser cut sheets, loose sheets of paper packaged similarly to a standard paper. Medicare will accept this Version 02/12 only.
These revised CMS-1500 02/12 forms, the only current accepted CMS-1500 version, for private practice providers and anyone billing within the non-institutional medical industry contain the following features: 
·        Accommodates reporting of the National Provider Identifier (NPI)
·        Designed form processing on high speed OCR Scanning equipment
·        Printed in special red ink
·        20# paper
